DWI Treatment Services:

Should treatment be necessary, we provide full treatment services as well.

What to bring to your appointment:

* Driver's license number and Photo ID.

* Driver's abstract.

* A copy of the police report.

* A copy of your DWI case history, if applicable.

* A copy of any court orders, citations, and/or breathalyzer report, if applicable.

* Two contacts' information: Family members, friends, coworkers, etc. that can attest to your drinking habits.
